In-Home Nursing Services for Medically Fragile Children Under the Age of 21
The Georgia Pediatric Program (GAPP) offers the opportunity of at-home nursing care and personal care to medically fragile children. Home Care Promise is a state-of-Georgia provider of professional, skilled nursing care to children in the comfort and safety of home.
GAPP In-Home Nursing Services
Skilled nursing and personal care services provided under GAPP include:
- Assistance with daily living activities such as bathing, meal prep, and housekeeping
- Monitoring vital signs such as blood pressure, pulse, respiration & temperature
- Assistance with ambulation & transfers
- IV therapies
- Wound care
- Tube feedings
- Gastrointestinal disorders
Is Your Child Eligible for Care Under GAPP?
GAPP is a Medicaid program for medically-qualified children under the age of 21. Children of families eligible for Medicaid can apply for GAPP at any time and must meet the same level of care necessary for admission to a hospital or skilled nursing facility. GAPP is managed by the Georgia Department of Community Health (DCH).
